Youth Weight Management Program Search
This program inventory collects detailed information on points of contact, goals, outcomes, location of facilities, and reimbursement and cost issues. If the acquisition of this data is not readily available upon entry, we recommend the user to print a copy of the questionnaire to facilitate the entry of the data. This entry program will only save your responses into the inventory database upon completion of all survey data. If any page remains idle for 30 minutes, all program data entered will be lost.
 
To enter a new program, a search of existing programs must be performed. To perform a simple search; select the county the program is offered in and select the ‘Execute Search’ button. To perform a more detailed search; program name or enrollment contact name may be entered, then select the ‘Execute Search’ button.
